HIPAA Security Risk Assessment

HIPAA security risk assessments that stand up to scrutiny

The HIPAA Security Rule requires covered entities and business associates to conduct an accurate and thorough risk analysis — and failing to do one is among the most common findings in federal enforcement actions. Healthcare also remains the costliest industry for data breaches. We deliver a documented risk analysis and a practical plan to reduce the risks it uncovers.

What our HIPAA assessment includes

Mapped to the administrative, physical, and technical safeguards of the HIPAA Security Rule:

  • Inventory of systems, vendors, and data flows that touch ePHI
  • Threat and vulnerability identification for each system
  • Likelihood and impact ratings with documented methodology
  • Review of access controls, audit logging, encryption, and transmission security
  • Business associate agreement and vendor review
  • Contingency planning, backups, and emergency operations review
  • A risk management plan with prioritized remediation

Prepared for what’s coming

HHS proposed significant updates to the HIPAA Security Rule in January 2025, including stronger expectations for asset inventories, multi-factor authentication, encryption, and recovery testing. As of September 2026, those changes are still proposed and not yet final. We assess you against today’s requirements and flag where the proposed changes would raise the bar, so you can plan ahead without overreacting.

Built by people who build healthcare software

Through our sister company, SwiftSoft Solutions, our team has engineered HIPAA-compliant applications and EHR integrations. We understand how PHI actually moves through modern health tech stacks — APIs, cloud services, analytics, and mobile apps — not just through a front-office network.

A note on independence: we prepare you for compliance and strengthen the controls behind it. We are not a law firm, CPA firm, QSA, or C3PAO, and we don’t issue certifications or attestation reports — we’re glad to work alongside the counsel and assessors who do.

FAQ

HIPAA Security Risk Assessment: common questions

Don’t see your question? Ask our security team directly.

Is a HIPAA security risk assessment required?

Yes. The HIPAA Security Rule requires covered entities and business associates to conduct an accurate and thorough assessment of risks to the confidentiality, integrity, and availability of electronic PHI, and to implement measures that reduce those risks. It should be reviewed regularly and updated when your environment changes.

Can you certify that we’re HIPAA compliant?

No one can. There is no official HIPAA certification. We provide an independent, documented risk analysis and remediation support that demonstrates a good-faith, ongoing compliance effort. Legal interpretation should come from your healthcare counsel.

How often should a HIPAA risk assessment be done?

HIPAA doesn’t set a fixed interval, but annual assessments are the widely accepted practice, along with updates after significant changes such as new systems, new vendors, office moves, or a security incident.

Do business associates need a risk assessment too?

Yes. Business associates — including software vendors, billing companies, and IT providers that handle ePHI — are directly subject to the HIPAA Security Rule.
